Remember that acceleration is a change in velocity per unit time, or a rate of change in velocity. So, velocity can be increasing while the rate of increase goes down.

Yes. An object traveling at a constant speed in circular motion is constantly changing direction and since velocity is a vector quantity it is also changing velocity.
